Starts at Chapin Woods from Chapin Ave. Goes through Reed's Basket Willow Swamp where the trail loops, and branches off to the exit by Annfield Ct, which goes to Westminster Ct. The trail goes along Circle Rd to Todt Hill Road. Turn left on Helena Rd and follow until the road ends and turns into woods. Take trail to, then through High Rock Park to Manor Rd, to cross into Moses Mtn Park. The Moses Mtn itself isn't part of the yellow trail, but can be climbed up via rock scramble for great views (these paths are on the greenbelt trails map). After going through Moses Mountain Park, cross Rockland Ave, to Meisner Ave, then London Rd, where there is an entrance to the woods. The trail goes west through Latourette Park to the multi-purpose trail. The multipurpose trail is taken south along the golf course, where an entrance into woods for the remainder of the trail. Trail exits onto Forest Hill Rd.

This trail is mostly well marked, but there are areas where it is lacking, such as around Heyerdahl Hill and through the forest toward the end.
